In general, it is recommended to ensure your game is not corrupted or otherwise damaged by verifying it's cache files. Valve has posted a very helpful guide.

General Team Fortress 2 issues
"Failed to create decoder for MP3" / Out of memory or address space. Texture quality setting may be too high.

There is a bug in TF2 that causes some sound files to not be loaded correctly into the sound cache. The game will attempt to play the sound over and over, starting a new instance of the MP3 decoder to play the MP3 file. However, they are never deleted and the old copies stick around. You will eventually crash with the above error. There's no known "permanent" fix but these steps should resolve it and you can repeat it whenever you run into the same issue.
You are having this issue if you see a lot of red lines containing: "Failed to create decoder for MP3".
  • Follow these instructions provided by Valve to open the properties menu.
  • Click on browse local files.
  • Click on the Windows search bar and type *.cache
  • Select all the cache files and delete them. They are safe to delete.
  • Optionally, verify your game cache.
Typing snd_restart when you see the Failed decoder messages in the console will temporarily avoid a crash, but it's a very short term fix and I'd recommend going with deleting the cache files instead.

"Map is missing" error
  1. Ensure that "Allow all custom files from server" is selected under Options > Multiplayer.
  2. Reconnect to the server and you should download the proper copy.

"Map differs from server" error

  2. Click on browse local files.
  3. Go into the tf folder then download folder. This is where any content you downloaded from servers are stored.

Freak Fortress 2
Everything is an error sign and/or purple and black checkers

  1. Refer to map is missing error

Some (and only some) bosses have missing textures/purple checkers, is a error sign or is completely white.
  1. Follow these instructions provided by Valve to open the properties menu.
  2. Click on browse local files.
  3. Go into the tf folder then download folder. This is where any content you downloaded from servers are stored.
  4. Delete the materials, models and sounds folder.
(If you know what you're looking for, you can delete only the problematic ones).
  5. Reconnect to the server to download the proper copy.

If even after these steps, that is ensuring that
  1. you have "Allow all custom files from server" is on under Options > Advanced
  2. you have tried the above steps
  3. you have verified your game cache using these instructions
Let us know and we'll investigate to determine if the problem is on our side.
